The starting position for all ICE-guided procedures. The catheter is in the mid-right atrium in a neutral (no rotation) position. Provides a clear view of the tricuspid valve, RV inflow, and interatrial septum — the reference landmark for all subsequent views.

Patient Position
Supine; catheter introduced via femoral vein, advanced to mid-RA
- 1Advance the ICE catheter via femoral vein to the mid-right atrium
- 2Position at approximately the level of the fossa ovalis
- 3Keep the catheter in neutral position — no anterior/posterior flexion, no rotation
- 4The tricuspid valve should appear on the right side of the screen
- 5The interatrial septum should be visible as a vertical structure

- Unexpected mass in RA
- Severe baseline TR
- Large pericardial effusion
